Referral
A referral is submitted by a participant, family member, support coordinator or provider using the online Referral form or by contacting Mirrored Mindset directly.
Intake and initial contact
The referral is reviewed and, if there is capacity, an intake call is arranged to clarify goals, risk, supports and whether Mirrored Mindset is the right fit.
Meet and get to know you
An initial meeting is held with the person and key people in their life to understand their story, strengths, needs, culture and what is important to them.
Assessment and planning
Assessment may include observation, information‑gathering and data review, leading to a Positive Behaviour Support plan with proactive strategies and clear guidance for the support team.
Implementation and training
Mirrored Mindset can provide training and coaching for families, carers and support workers so everyone feels confident using the strategies consistently.
Ongoing review
Progress is monitored, data is reviewed and the plan is updated so supports stay relevant and aligned with the person’s goals and safety.
